I can't seem to find comments.php listed on the hierarchy chart http://wphierarchy.com.
is that essentially b/c there is no fall-back for it?
I noticed a lot of files are missing from the hierarchy...I'm assuming that is for the same reason?

The standard comments template is the same for most themes and doesn't necessarily need a comments.php creating. You can reference an alternative if needed though.
