Always working with Smartphone layout when adding elements

Considering this tutorial taught me how to create a website by first designing the mobile version of a website, If I wanted to add to the website, such as an image to the left side of the header 'Nick Petite", would I need to design it so that it works on the mobile dimensions and then mess with media queries to fix the desktop versions?

I tried adding an image to the right of the name paragraph in the header but had difficulty as the navigation block just disappeared and the image simply appeared below the paragraph.

Can you post your code. It's very possible that if you are using floats to align it your container is collapsing. If this is the case you would need to add a clear fix. I recommend you post your code or a link to show us what is happening.
